I kind of ripped this topic from a recent GameSpot article (Worst Selling Consoles of All-Time). This topic/poll is for US releases only.
My list, with #1 being the worst:
10. Nuon-enabled DVD game systems (Toshiba SD-2300, et al.)
09. Philips CD-i
08. Commodore C64 GS
07. Amiga CD32 (1993) - this did OK in the UK release
06. Memorex VIS (1994)
05. Commodore CDTV (1991)
04. Bandai Pippin @World (1995)
03. Pioneer LaserActive (1993)
02. Capcom CPS Changer (1994)
01. RDI Halcion (1984) - hands down the biggest failure of all-time

I think your list is solid, but I think the list needs to be put in context.. Seems like most of your list failed because of absurdly expensive hardware or games, especially in the early to mid 90s when companies were trying their hands at CD-based consoles. They were absolutely still failures however.
I think most people identify 'failure' with systems like the horrendous run that Sega had (Sega CD, 32X, Saturn, Dreamcast) or the Nintendo 64 which, while successful, they are still feeling the effects of.
