BBFC is no longer used for rating video games in UK. Germany will switch to PEGI exclusively in 2017.

The original Darius (Which is a "Arcade Archives" release on PSN) is getting a physical release but it's only available in a big box set.

Toro (PS4): Only released physically in Spain.
Los Caballeros Del Zodiaco Alma De Soldados (Mexico) / Os Cavaleiros do Zodíaco: Alma dos Soldados (Brazil) (PS4): Only NTSC releases of Saint Seiya: Soldiers' Soul. Not released in US/Canada.
Atrapa la Bandera (PS4): Only released in Spain.
Rugby League Live 3: Released in PAL regions only.
Dino Dini's Kick Off Revival: Released in PAL regions only.
These were all submitted recently by hockeycollector. Thanks!
Also:
Yomawari: Night Alone was recently released on Vita.

It's being sold as Yomawari: A Night ALone + htol#NiQ: The Firefly Diaries (both games on the same card). Comes with a reversible cover slip so each game has its own cover. Thanks, NIS - I've been wanting an English version for a while!

Some info about Games Workshop licensed games:
Space Hulk Ascension (PS4) - Available physically in PAL regions
Warhammer 40,000: Deathwatch (PS4) - already on the list, but isn't due for physical release until February 2017.
Also:
There are 2 Tadeo Jones games for the Playstation Vita. The second is:
Tadeo Jones y el Manuscrito Perdido (PSV) - Spain only.

If we're going to that depth, the Muramasa demo for Wii was only distributed by Play-n-Trade and most were thrown out.

Figured I'd chime in here, since I've been submitting some of these recently and I've been keeping up similar lists (exclusively for PS4) on some other forums.
Pure Hold'Em World Poker Championship
(PS4) is one I just submitted on here. As far as I know it is only available through Wal-Mart.com and a few ebay sellers.
Tumblestone
is on this list and but I'd add that, for now, it appears to be an NTSC exclusive release, which is a bit odd for that type of game. It may be worth adding
Atari Flashback Classics: Volume 1
and
Volume 2
(PS4) to this list, seem to be NTSC-only with some odd distribution channels (i.e. Amazon already has ran out of its own stock).
Got confirmation from Sodesco directly that
Tower of Guns
for PS4 and XB1 will see a retail release this month in the U.S., its already appearing on some online inventories. Got another direct confirmation from Headup Games that
Bridge Constructor
(PS4) will release at the end of November in physical format in German-speaking countries only. Also UIG has let me know that
Professional Farmer 2017
(PS4, XB1) will see a physical release in the U.S. in a matter of days (it may already be trickling onto Wal-mart shelves, someone saw an XB1 copy the other day). I have a feeling that release will be really under the radar. According to Microids
Moto Racer 4
will see a stateside release on January 24th, I'm assuming it will be a physical release but we'll see. I did pick up the PAL copy already (came out last week). These are all direct messages I've gotten from the publishers on either Twitter or Facebook, so the info is right from the source.
Other German-exclusive titles hitting late November/early December are
Silence
and
Der Industrie Gigant II HD
(Industry Giant II). Silence looks like a game that maybe could get published in other regions over time, but it is a German developer/publisher working on that, so I'm not sure what the plan is with that game...
