I went to pull my Rock Band disc out of the case and I guess I bent it too hard and cracked the disc on the inner circle so it barely reaches the data.  The game plays most of the time but every once in a while I get a disc error and kicked back to the dashboard.

some disc cases are ridiculously hard to get the disc out, i'm always scared i'm going to do something like you did.
