Someone explain Every Extend Extra Extreme to me

It seems like the only way you can lose the game is if you actively try. I just finished a 40 minute round on the radar level, and near the end I actively avoided +time powerups just to get the game to end.

Is there any difficulty in this game?

Damn, I must really suck at this game. I just don't think the game clicks at all for me, I kept losing on the first level and I gave up, but if it plays like you say then I ought to give it another go!
