I've had this one for a while but I'm just now getting around to asking.
It's entitled "Baseball" but the one in the database here does not look anything like it.  It does look similar to the Super Challenge Baseball by M-Network, but the game does not say Super Challenge.
Anyway here's a picture of the game in question.

Also, there is no title screen, it goes right into the game if that helps.

that would be Super Challenge Baseball from Intellivision, Inc. (later bought out by Mattel)

The shape is like an old Intellivision cartridge that the company simply modified to fit the 2600.  I think this needs to be added to the database.  The M Network version of this game has the full title on the end, but in blue text on a black background, and the Telegames version is more like a normal 2600 cart with a big ugly white label on the side.

Hey, in an earlier thread they decided that they are considering the INTV games separate from the M Network releases, so go ahead and add it as Super Challenge Baseball.  The addition will in turn be credited to your name.
