Not an expert about cart variance personally, but the 2nd (from the database), looks like a game genie or Sonic&Knuckles, in that another cart would fit into it.  I know the old NES game genie had those plastic flaps to support the other game...or something.

Hope that helps.

its possible that it was made kinda like super noah's arc for the snes to fit a game into it to bypass the lockout chip.

It is commercial and at the time along with Tetris was a popular thing. correct me if I am wrong but there was a Micromachines cartoon on???

I myself got brainwashed and convinced mother to buy me one of those carwash Micromachines for my cars...which was kind of dumb to begin with since the cars rust after being in water >:(

It is defiantly  a cart variant, and you should submit it as such. Nice find, by the way!  ;)

Here you go:

When you get the the part where you add the Variation Title which is underneath the game title, you should name it something like Black cartridge / Picture Label or something fairly descriptive like that. So when people search the database and these two variations of the same game show up, they pick the one that's suitable to what they have.

 :)

Nice! That cart looks unlike anything I've every seen! (NES-cartridge-wise of course :P)

Fantastic Adventures of Dizzy on the NES had the same cartridge. I think it's something to do with the Aladdin Deck Enhancer.
